HOW IT WORKS

An  egis coordinator meets with each member to assess both individual and family needs related to preserving independence and improving health, safety, and quality of life. This confidential assessment enables the care manager to respond knowledgeably and promptly to the member's current needs and to establish a confidential file for future needs.

When the member requests assistance, egis staff will provide information or arrange for appropriate services and follow-up to ensure that they are satisfactory. The role of egis is to plan for and coordinate services, be a source of information, and be an advocate to see that the member's needs are met.

PAYMENT OF SERVICES

 There is an initial assessment fee of $300 per person when the member joins  egis. This fee pays for the initial meeting between the care coordinator and each member and the subsequent creation of a plan of care based on the   member's needs. The annual reassessment fee (on the anniversary of joining egis) is $175 per person.

Requests for coordination of services or information after the initial assessment will be billed to the member at a rate of $72 per hour (in 15-minute increments) based on time spent securing services or information

SERVICES

egis staff have knowledge of community resources and extensive experience with the organizations and professionals in the Santa Fe area that members may need. Some of the services that egis coordinates include:

Y Private home health aides: short-term and round-the-clock

Y Planning and management of support during hospitalization, at time of discharge, and at home

Y Transportation and drivers

Y Meal support services

Y Handyman services

Y Legal and advance directives

Y Private RN services

Y Nutrition consultation and meal planning

Y Gardening and yard work

Y House and pet sitters

Y Dog walking and pet care

Y Housekeepers

Y Physical therapists

Y  Personal organizers

Y  Funeral/cremation planning and services
